How to get Mayor's Permit

Here are the steps on how to apply for NEW/RENEW Mayor's Permit:
 
1. DOWNLOAD AND SECURE THE APPLICATION FORM.
 
2. BRING THE FOLLOWING REQUIREMENTS:
 
  • Brgy. Clearance
  • Xerox of Residence Certificate or Corporate Tax
  • Xerox of DTI / SEC Registration
  • Health and Sanity Permit
  • Certificate of Annual inspection
  • Xerox of BIR form 0605
  • Certificate of Philhealth

3. ASSESSMENT (Business Permit Licensing Officer at Mayor's Office)

4. ASSESSMENT AND PAYMENT (Bureau of Fire Protection and Office of the Municipal Treasurer)

5. SIGNING OF MAYOR'S PERMIT BY THE MUNICIPAL TREASURER AND MUNICIPAL MAYOR THEN RELEASING
